Scheduling a Follow-Up Visit

Medical Records

For copies of your medical records, we need a completed and signed Medical Release Form. Click Here to print the form. Then, please fax the completed and signed form to the appropriate office above:

 

Billing

We accept most major insurance plans. However, pre-authorization may be required for many of the treatments administered by The Los Angeles Cancer Network. If you are unsure about your coverage, please call our office or see our page under insurance coverage.

If you are not covered by health insurance for your visit or any of the treatments we provide, we may be able to assist you with costs or make special financial arrangements for our services.
