- This is the full registration packet for new patients, including the forms below.
- It is a fillable PDF, meaning you can it fill it out online. But once it is filled out, it needs to be downloaded to your computer.
- Once downloaded, you can print it out and bring it to your appointment or you can email it directly to newpatient-group@lahomg.com with your name in subject line.
- This packet is only complete once it is either emailed (newpatient-group@lahomg.com) or printed out and given to our staff.
